What is voluntary active euthanasia?

The Stanford Encyclopedia of Philosophy, in an entry last updated on Feb. 19, 2007, defined voluntary euthanasia in the following terms:
"Those instances of euthanasia in which a clearly competent person makes a voluntary and enduring request to be helped to die."

Erich Loewy, MD, Professor of Bioethics at the University of California, Davis Medical Center, in his 1998 essay "Harming, Healing and Euthanasia," published in Regulating How We Die, defined active voluntary euthanasia as:
"A direct act of killing, done at the direct request of the person being killed."

Michael Manning, MD, Author, in his 1998 book Euthanasia and Physician-Assisted Suicide: Killing or Caring?, defined voluntary active euthanasia as:
"Intentionally administering medications to cause the patient's death at the patient's request and with full, informed consent."
